THE DOG SITTER, LLC TERMS AND CONDITIONS 1. These Terms and Conditions are entered into by and between THE DOG SITTER, LLC (TDS) and myself. 2. I, the Client (pet/home owner), authorize TDS to perform care and services as outlined in the Client Account. 3. In the event of personal emergency or illness of my agreed upon Pet Care Professional(s), I authorize TDS to arrange for another qualified person to fulfill responsibilities as set forth in this contract. Client will be notified in such case as soon as possible. 4. I (or my homeowner’s insurance) will be responsible for any injury (including without limitation to pet bite, scratches requiring medical attention, etc.) to my Pet Care Professional due to my pet(s) or condition of premises where my pet is located. 5. TDS and its independent contractors agree to provide the services stated in this contract in a reliable, caring and trustworthy manner. In consideration of these services and as an express condition thereof, I expressly waive and relinquish any and all claims against my Pet Care Professional and TDS except those arising from negligence or willful misconduct on the part of my Pet Care Professional. 6. TDS is fully bonded and will not be responsible for loss, theft or damage to client’s property. 7. I understand an initial consultation is no more than thirty (30) minutes. There is no charge if said consultation occurs three (3) days or more before first visit. I understand if my initial consultation occurs within three (3) days of my first visit, I will be charged $20. 8. It is my responsibility to ensure my scheduled visits are accurate. I can verify my visits by logging into my client account and navigating to the “schedule” page. 9. I understand booking requests and cancellations will be scheduled through my client account, which can be accessed at https://www.timetopet.com/portal/thedogsitterlv or from the Home page of www.thedogsitterlv.com . 10a. Vacation Client Non-holiday Bookings. I understand the amount owed for a service is due the day prior to my pet’s first visit. However, if the amount owed is more than $300, 20% down is required at the time of booking, with the remaining balance due the day prior to my pet’s first visit. I understand there is no credit/refund for early returns or cancellations made within 48 hours of first visit of a service, and I understand I will be immediately responsible for the balance. EXCEPTION: If I have purchased twenty (20) or more visits in the last twelve (12) months, I will receive credit for up to three cancelled visits. NOTE: If payment is not made by the due dates, service will be cancelled. 10b. Vacation Client Holiday Bookings. I understand the amount owed for a service which falls on a major holiday (Thanksgiving, Christmas, New Years Eve) is due fifteen (15) days before my pet’s first visit. However, if the amount owed is more than $300, 20% down is required at time of booking, with the remaining balance due fifteen (15) days before my pet’s first visit. I understand there is no credit or refund for early returns or cancellations made within seven (7) days of first visit of a service. NOTE: If payment is not made by the due dates, service will be cancelled. 10c. Daily Client Bookings (3 or more visits weekly on a monthly basis). I understand my first month of service is due before my pet’s first visit. Thereafter, if I have a credit card on file in my client account, payment is due the last day of the month in which visits occur. If I do not have a credit card on file, payment is due the first day of the month. I understand there is no credit/refund for cancellations made after 7 am of the day of the visit. EXCEPTION: I will receive credit for up to one cancelled visit a month. NOTE: If payment is not made by the due dates, service will be cancelled. 11. I understand if I choose to mail a check for service, that it must reach TDS the day before my pet’s first visit. TDS does not accept checks left at the home, unless otherwise arranged with TDS. I understand a finance charge of $15.00 will be added to overdue balances after thirty (30) days and a handling fee of $25.00 will be charged on all returned checks. No further visits will occur until overdue balances are resolved. 12. My Pet Care Professional may contact the pet owner if additional care is needed or recommended. The client will have authority to accept or decline these requests. If the homeowner or emergency contact is unable to be reached, my Pet Care Provider will perform the recommended care for the pet(s) and I will be invoiced for said visits. 13. I understand requests for a same day visit is the standard rate plus $10. 14. I understand the fee as outlined on my invoice(s) is for agreed upon visit(s) to my home, not including any additional expenses or fees that may be added to my invoice. 15. I give permission to TDS to take photographs or videos of my pet(s) to use for marketing use. 16. I further authorize this contract to be valid approval for future services permitting TDS to accept my reservations for service and enter my home without additional signed contracts or written authorizations. An updated Client Account must be executed if I move, if I obtain additional pets, or if I have updated instructions regarding the care of my pet(s) and/or home. 17. While TDS will make every effort to accommodate my pet’s schedule, block scheduling is utilized and specific visit times cannot be guaranteed. Exceptions may be made for potty breaks, senior pets, special needs, and pets requiring medication at specific times. DESIGNATED BLOCKS: Morning 6AM–9AM Afternoon 11AM–2PM Fourth Visit Only 4PM-5PM Early Evening 5PM–7PM Late Evening 7PM-9PM 18. I will provide two methods of entry to my home to TDS. I further understand that all forms of entry must be presented at initial consultation or dropped off at The Dog Sitter offices five days before first visit. In the event that TDS cannot get into my home with the keys or remotes I provided, I agree to allow a locksmith to provide entry into my home at my expense. Additionally, I understand that The Dog Sitter will retain said methods of entry until I request them to be returned. At that time, I am responsible for picking up the keys/remote from The Dog Sitter. There is no charge for key pick-up/drop-off at initial consultation. 19. If adequate supplies are not left and my Pet Care Provider has to purchase supplies, a full reimbursement and an $18.00 charge will be added to my invoice. 20. Both pet sitting and dog walking visits, unless otherwise agreed to in writing, are 25-30 minutes in duration and potty breaks are 10-15 minutes. TDS does not perform dog walking duties at night. I can contact TDS and request pricing for extended visits. 21. Other than an emergency, communication between my Pet Care Professional and myself will occur through my client account. Updates consisting of a daily checklist, brief messages, and/or photos may be viewed via my email. I may respond directly to any of the emails and will receive a reply from either the Pet Care Professional or the offices of TDS. 22. TDS accepts no liability for any breach of security or loss of or damage to the Owner’s property if any other person has access to the property during the term of the agreement. 23. I have reviewed these Terms and Conditions for accuracy and understand and agree to provisions contained herein.
TDS VETERINARY MEDICAL CARE RELEASE 1. In the event of a medical emergency requiring the services of a veterinarian, TDS will attempt to contact me by phone. If neither I, nor my emergency contact, can be reached in a timely manner to make pet care decisions, I understand my Pet Care Professional will defer to the veterinarian's protocol. 2. I authorize emergency veterinary care for any of my pets listed in my Client Account, releasing TDS and its independent contractors from all liabilities related to transportation, treatment, and expense. 3. I give my express permission to take my pet(s) to the veterinarian I listed in my account or to the closest open facility if the primary vet is unavailable. 4. I understand there is a $50.00 fee for each emergency visit to the veterinarian. TDS does not pay for any expenses incurred. It is my responsibility to make payment arrangements with the veterinarian.
